You hereby authorise the Fysisch-Mathematische Faculteitsvereniging to send recurrent collection instructions (direct debit, incasso) to debit your bank account the yearly contribution and any costs made in the streepsysteem or during activities. You authorise your bank to debit your bank account on a recurrent basis in accordance with the instructions from the Fysisch-Mathematische Faculteitsvereniging.
